     To utilize the on-line web study week conference registration form, click below (on View Conference Registration Form).  This form is in Adobe Acrobat format.  To view and print the form, you need to have Adobe Acrobat Reader, a free download from Adobe Systems, on your computer.  If you don't have Adobe Acrobat Reader, click first on the "Obtain Adobe Acrobat Reader" link below, to initiate download and installation.  Following that, or if you already have the Adobe Acrobat Reader, click on the link "View Registration Form".
     When the form web page comes up on your browser, print it to your printer.  Fill out the form and mail it with check included to the address shown on the form.

Scholarships

 
Background The SWLC has set aside funds to provide scholarships covering registration to the study week for participants from dioceses of the region who might not otherwise be able to attend.
Application Forms Contact the Office of Worship or Liturgical Commission of the applicant's (arch)diocese for a scholarship application form.
Award Process Scholarships will be awarded to qualified applicants, on a first come / first served basis.
Where to Send Forms Completed scholarship application forms should be mailed to the applicant's (arch)diocesan Office of Worship or Liturgical Commission.  DO NOT send the scholarship application to the SWLC 2005 Study Week registration office.

Hotel Reservation - Process

 
Method of Reserving
    Hotel Room:		 Mail your completed reservation form with either credit card info on form or check attached to the Adam's Mark (see information below).

Reservation Deadline: Late reservations will be subject to availability.
Other Terms:

At time of check-in, please bring a copy of your Tax Exemption Form, if applicable (only available to Texas churches and dioceses).  Reservations should be made in the name of the church of diocese and you must pay at checkout with a parish or diocesan check or card.

Reservation forms must be accompanied by a deposit equal to one night’s room rate plus 15% tax.  This deposit will be applied to the last night of the reservation. Deposits made via a credit card will be considered guaranteed and billed accordingly in case of a no-show.

In the event that you do not arrive as scheduled, cancel reservations less than 72 hours prior to my scheduled arrival, or depart earlier than the date scheduled. you are liable for one night’s room rate plus 15% occupancy tax, which will be covered by your deposit.

American Airlines has offered us a discount on reservations made 30 days in advance and reduced discounts for reservations made less than 30 days in advance. Discount information is available when you call 1-800-433-1790. When calling that same number to make your reservation, please use our study week authorization code A4215AE.

Super Shuttle has given us a flat discount rate of $13.00 each way from either DFW Airport or Love Field Airport. (This does not include gratuity.) You will need to mention the Southwest Liturgical Conference for this rate, otherwise you will pay $45.00 each way. You can make your reservation from the SWLC website or the Super Shuttle website ( www.supershuttle.com ) or you may use the coupon printed in the registration brochure. A copy will also be acceptable. You will need two coupons, one for traveling each way. Call 1-800-258-3826 for more information.

If you live in the Dallas/Fort Worth metroplex, we suggest riding the DART rail for your convenience. There is a station right next to the Adam’s Mark. Day passes for bus/rail are $2.50. Go to www.dart.org for the routes from your location.
